The substitution of Leah for Rochel

Some questions on the substitution of Leah for Rochel.

Why was Leah's marriage to Yaakov binding--wasn't it a mekach to'us? And even if you want to say that after he found out, he consented (so as not to embarrass Leah), still, at the time of the ceremony, he thought it was Rochel, so it would seem that the marriage only took effect the next morning. This would mean that during the first relations that night, when Yaakov thought it was Rochel, he was having relations with a woman to whom he wasn't married? That seems very difficult to say.

It is forbidden to have in mind a different woman during relations. How could Leah have had relations with Yaakov, knowing that Yaakov thought that she was Rochel? Couldn't she have told him once they were alone, or done something? Wasn't Leah causing him to commit this sin, albeit be'oines? For that matter, since Rochel gave the simonim to Leah, wasn't she also being machshil Yaakov in having the wrong kavono?

Both of the above are especially hard to understand since we know that Re'uven was born from this first union--"reishis oini".

Also, how could Yaakov have been nichshal in this; what about "lo ye'uneh l'Tzadik kol oven"?
